New Patients
When you make an appointment at Seven Circles Natural Medicine you will meet with your scheduled provider for an hour-long appointment. During that time you will receive a comprehensive evaluation and treatment. You may also receive nutrition, acupuncture, and/or lifestyle counseling and be prescribed something from our natural pharmacy.
To schedule your first appointment, please call our office so we can get you set up in our health records system. If you'd like to use insurance please have your card ready, as we will need some information to verify coverage. Please note: we encourage our patients to be familiar with their own policy as you are ultimately responsible when coverage is denied.
Before your first visit with us, we will send New Patient paperwork, consent, and medical forms to you via DocuSign. You will also be invited to set up your patient portal for future communication and HIPAA-compliant document sharing. It is required that all paperwork be completed at least 24 hours prior to your scheduled time. Please also eat and drink water before your appointment and if you plan on receiving acupuncture wear loose-fitting clothing that can be pushed above the elbows and knees.
If you are taking any prescription medications or vitamins/supplements please make note of them in the New Patient paperwork.
Cancellation Policy: We require at least 24 hour prior notice for all appointment cancellations. Unless there is a legitimate emergency, cancellations made the same day of the appointment and no-shows will be charged $120 for both ND and LAc appointments. The fee will be waived if we can fill the appointment. For ND visits the appointment can also be converted to telemedicine for the same day and time.
